Terms and Conditions

Revitalize Wellness Affiliate & Practitioner Program Terms

1. Joining. Every application is reviewed. We may approve, decline, or remove any partner at our discretion. Licensed practitioners must provide a valid credential and an active practice.

2. Programs. You'll be placed in one program when you're approved: Affiliate, Practitioner, or Store Credit. Your commission rate, customer discount, and payout method are shown in your partner dashboard. We may change rates or move you to a different program with 30 days' notice by email.

3. How you earn. You earn commission on qualifying orders placed through your link or code. Orders that are refunded, cancelled, or charged back don't qualify, and any commission already paid on them may be deducted. Your own orders don't earn commission.

4. Affiliate review. Affiliate programs are reviewed every quarter, and terms may be adjusted based on results.

5. Payouts. Commissions are reviewed and approved monthly. Cash payouts go through PayPal once your approved balance reaches $25, and smaller balances carry over. Store Credit partners are paid in store credit, which has no cash value.

6. Promotion rules. You may not:

  • Bid on "Revitalize Wellness" or any variation of it, our product names, or misspellings in Google Ads, Bing, or any other paid search
  • Post your code on coupon or deal sites
  • Make medical claims, or say our products diagnose, treat, cure, or prevent any disease
  • Imply that you're Revitalize Wellness or speak for us
  • Send spam or use misleading promotions

7. Disclosure. You must clearly disclose your relationship with us wherever you promote us (for example, "#ad" or "I earn a commission"), as FTC guidelines require.

8. Practitioners. You're responsible for following the rules of your license and your state, including any rules on accepting commission. If you can't accept commission, tell us and we'll set up a patient-discount-only option.

9. Ending. Either side can end the partnership at any time. If you break these terms, unpaid commissions may be forfeited.

10. Changes. We may update these terms. If you keep promoting after an update, you're accepting the new terms.
